Even when everyone knows there are horrible things going wrong it's almost impossible to solve the problems because the system works to protect itself before serving its "customers". This is the essence of "big government." Nothing is precise. Few are accountable. The key is to go along to get along. Everyone clings to their job until the payoff of retirement.
